How can we keep medical care accessible to everyone throughout their whole life? What is the most effective strategy for preventing depression following a myocardial infarction? How will we cope with the disease and financial burdens presented by obesity and diabetes mellitus? What kind of tailored care will be offered to the growing group of elderly patients? How can clinical research contribute to medical guidelines and protocols? How can we keep care personal and focused on the individual, despite guidelines and protocols? This Learning Community the personal story of the patient is an important factor in the gathering of knowledge about disease and health. We investigate how this story can help us provide high-quality medical care. We focus on medical care characterized by continuity, effectiveness, timeliness, safety and affordability – in short, sustainable care. This LC thus concentrates on the way that medical health care will be shaped after 2025. If you want to help think about the future of sustainable care and the role you can play, this is the option for you. This LC is Dutch-speaking.
